There is no error message for this one, only the symptom in the title.
Cause
The Mac clock was 81 seconds slow. The local API set expires_at = now + 25 s by its own clock, which was already in the past by the database's clock.
Fix
Resync the clock (sudo sntp -sS time.apple.com), and prefer database time (now()) for expiry timestamps over app server time.
